Rules for receiving diploma and supplement:

"former" graduate – after prior arrangement of the date of collection of documents (please make an appointment by e-mail or phone),
"current" graduate – upon receipt of notification,
personal collection during office hours,
in special situations, it is possible to collect the documents by an attorney after granting the power of attorney – the document can be downloaded from this page – Department of Studies,
it is required to present a valid identity document with a photo and a clearance slip confirmed in USOS,

Fees:

20 PLN per item – for issuing a duplicate: of a diploma, of a diploma supplement;
26 PLN per item – for the certification of documents issued in connection with the studies or completion of studies (transfer of records, diploma, supplement) intended for legal transactions with foreign countries.
