Rules for collection of the diploma and supplement:

"former" graduate – after prior arrangement of the date of collection of documents (please make an appointment by e-mail or phone),
"current" graduate – upon receipt of the information,
personal collection during office hours,
in special situations, the documents can be collected by an attorney after granting the power of attorney – the document can be downloaded from this page –> Department for Studies,
a valid identity document with a photo and a clearance slip confirmed in USOS must be presented upon the documents collection,

Fees:

PLN 20 per item – for issuing a duplicate: of the diploma, of the diploma supplement;
PLN 26 per item – for the certification of documents issued in connection with the course or completion of studies (transfer of records, diploma, supplement) intended for legal use abroad.
